COMPETITOR LIABILITY WAIVER

Wing Chun & Escrima International LTD. Waiver for the “1st Open Black Belt World Championship” in Toronto, Canada – Sifu Werner Leuschner.

By checking the box below, you agree, warrant and covenant as follows:

I have had previous martial arts instruction, and I understand that participating in the 1st Open Black Belt World Championship (August 18 – 19, 2007) may be dangerous and that I may be seriously injured, and I hereby assume all risk of physical and mental injuries, losses, or disabilities, which may result from or in connection with my participation in the 1st Open Black Belt World Championship. I freely waive all rights to Sifu Werner Leuschner, the instructors, and staff of Wing Chun & Escrima International LTD., the World Martial Arts League and to the Economic Development, Culture & Tourism.

I also understand that participation in the 1st Open Black Belt World Championship, especially but not limited to light contact or full contact sparring matches, entails a great risk of injury, and I assume full responsibility for all my actions, intentional or otherwise, during and in connection with my participation in the 1st Open Black Belt World Championship.

I do hereby release, now and in the future, Wing Chun & Escrima International LTD., World Martial Arts League and Economic Development, Culture & Tourism, as well as their officers, representatives, servants, agents, employees, volunteers, and all other related members from all claims, actions, suits, and controversies at law or in equity, by reason of any matter, cause or thing whatsoever that I may sustain as a result of or in connection with my participation in the 1st Open Black Belt World Championship, by myself, my heirs, personal representatives, and assignees.

I have accident and/or medical insurance to cover any injury that I might sustain; therefore, in return for admission to the 1st Open Black Belt World Championship, I forever give up any rights against Wing Chun & Escrima International LTD. and World Martial Arts League; their promoters, instructors, and staff, as well as to the Economic Development, Culture & Tourism.
I fully understand that any medical attention or treatment afforded to me by Wing Chun & Escrima International LTD., its officers, agents, representatives, servants, employees, volunteers, and all other related members will be of first aid type only that I accept and take willingly, and I hereby release Wing Chun & Escrima International LTD., its officers, agents, representatives, servants, employees, volunteers, and all other related members from any liability for such aid.

I further agree that my performance, attendance, and participation at the 1st Open Black Belt World Championship may be filmed or otherwise recorded. I consent to Wing Chun & Escrima International’s LTD. use of my name, likeness, voice and biographical data concerning fully or in parts, in any form or language, with or without other material, without limitation, for television, radio, video, theatrical motion pictures, or any other medium by devices now known or hereafter devised, and I do hereby waive any compensation in regard thereof as well as any future rights to the aforementioned.

I have read and fully understand the above waiver. I also understand that all fees paid to Wing Chun & Escrima International LTD. are nonrefundable. Parent or legal guardian is required if competitor is under eighteen (18) years of age.

If any provision of this Liability Waiver shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Liability Waiver and shall not affect the validity and enforceability of any remaining provisions.
